Yes! I often buy stuff like shoes and gloves, and some of my pieces are modified from thrifted clothes, but the vast majority of each cosplay is made from scratch by me. If you're curious about a specific costume, I have constructions notes in my gallery.

I try not to have "dream cosplays" anymore, because when I put a costume on a pedestal like that, sometimes I get too scared to make it. If I want to make a cosplay that badly, I make it!
My mom sews, so I learned to hand sew when I was very young, and started to learn the sewing machine in elementary school. It wasn't until high school that I really started sewing regularly, though.
Codex, from The Guild! Which, now that I think back on it, was a really complex costume to choose for my first one. I'm kind of still shocked it came out so well. Beginner's luck?
This varies so much by costume that it's basically impossible to answer. Some more complex costumes, like Royal Milk Tea, took 100+ hours, and some simpler costumes, like Cinderella's peasant dress, were done in a single day.
